Referrals
We accept referrals from:
- General Practitioners
- Consultant Psychiatrists/Consultant Forensic Psychiatrists
- Consultant Neurologists
- Case Managers
- Clinical Psychologists
- Forensic Commissioners
- Social workers
- Courts/Prosecution and Defence Solicitors
- Prison Service
- Probation service
- Other healthcare providers
- Specialist solicitors in mental health/brain injury
- Maximum Security Hospitals
- Medium Secure/Low Secure Units
- Other Hospitals
Upon receipt of a Referral, a Multi Disciplinary Team assessment will be carried out within two weeks of the referral being made, if possible.
The assessment will result in a comprehensive MDT report in a standardised format, which includes a section on risk management and an initial MDT care plan, the referral will also identify whether the patient is suitable for the service, if not a copy of the report will be forwarded to the patient's referrer informing them of the teams decision.
If the patient is suitable for our services a copy of the assessment report with a detailed Care Plan will be forwarded to the appropriate parties and upon agreement of funding a date for transitional work and transfer will be set.
The Commissioners can request a Gate Keeping Assessment in keeping with their referral and funding protocols.
Once transferred a further more in depth assessment is undertaken over a 3 month period to meet the specific health and social care needs of the patient. The patient is fully included in this process and encouraged to identify their own needs, treatment goals, targets and preferences.
