Model of Care
St George Health Care Group provides specialist neuro rehabilitation using a holistic approach. The MDT assesses each person's needs: physical. Psychological, social, emotional and occupational and MDT care plans are developed during the 12 week assessment period. Many patients are admitted with behaviours that are unhelpful to themselves and present a risk to others. The aim is to modify such behaviours using a combination of environmental structure, cognitive rehabilitation and a neurobehavioral approach. Baseline outcome measures are completed during the assessment phase and repeated at regular intervals to demonstrate change and inform further rehabilitation goals.
